  • My first language isn’t English. 

  • I want someone to go with me to my appointment for support. 

  • I want or need help filling out paperwork.

Why should I get

preventive screenings?

Preventive health screenings are important. They matter because they allow you to be proactive with your health and help you recognize the early stages of a disease. These screening programs are here for you so you can be there for them. You have more options than you think.

Your healthcare options, simplified

If you have low to moderate income you can apply for Medicaid Expansion/Heritage Health Adult. If you don’t meet the income requirement, you can apply for the Every Woman Matters Program and The Nebraska Colon Cancer Screening Program to get access to free services.

  • Medicaid Expansion/Heritage Health Adult

    Heritage Health Adult (HHA) is Nebraska’s Medicaid Program. You’re eligible for this program if you’re between the ages of 19-64 and if your annual income is $18,754 for just you or $38,295 for a household of four.

  • Every Woman Matters

    Every Woman Matters (EWM) is a federally funded program that pays for office visits for Pap tests, pelvic exams, clinical breast exams, mammograms and lab fees for women. You’re eligible for this program if you’re a woman between the ages of 35-74 and if your annual income is $36,450 for just you or $75,000 for a household of four.

  • Nebraska Colon Cancer Screening Program

    The Nebraska Colon Cancer Screening Program (NCP) is a colon cancer screening program that offers enrollment, screening, and education to Nebraska men and women who are 45-74 years old and if your annual income is $30,578 for just you or $62,438 for a household of four.

Over 44% of people don’t get preventive health screenings because they think they can’t afford the screenings or don’t have insurance.

Most health plans cover preventive health screenings. If you don’t have insurance, there are still ways to access these services, even for a limited time. These programs are here for you so you can be there for them.

Heritage Health Adult

Heritage Health Adult (HHA) coverage includes services like doctor’s office visits, hospital visits, prescription drugs, dental, vision, and over-the-counter drugs, plus comprehensive physical and behavioral healthcare. 

You’re eligible for this program if:

  • You’re between the ages of 19-64

  • You fall within this income level:

Every Woman Matters (EWM)

When enrolled in this program, you may receive the following breast and cervical cancer screening services:

  • Pelvic exam

  • Pap test

  • Clinical breast exam

  • Screening mammogram

  • Heart disease and diabetes screening

You may be eligible for this program if:

  • You’re female

  • You don’t have insurance

  • You’re 35-74 years old

  • You’re a U.S. citizen or permanent resident

  • You fall within this income level:

To begin the enrollment process, fill out the Healthy Lifestyle Questionnaire below to determine what services are best for you. Be sure to answer all questions to the best of your ability. Skipping questions could delay your health screenings. 

If you’re under the age of 35 you’re not eligible for the screening program. But, you may be eligible for the EWM Diagnostic Program or the State Pap Plus Program if you’re needing further diagnostic services (diagnostic mammogram, ultrasounds, breast biopsies, colposcopies, etc.).

Nebraska Colon Cancer Screening Program (NCP)

When enrolled in this program, you can receive the following services:

  • Home based screening test and/or colonoscopy. All screenings are based upon family and personal history and must be pre-approved by NCP staff. 

  • Colonoscopy following a positive home based screening test.

  • Review of health history and education about colon and rectal cancer screening.

  • Colonoscopy as initial screening test if determined to be the best screening test based on health history. 

You’re eligible for this program if:

  • You’re male or female

  • You don’t have health insurance

  • You’re between the ages of 45-74

  • You are a U.S. Citizen and Nebraska resident

  • You fall within this income level:

You can enroll in the Nebraska Colon Cancer Screening Program (NCP) by filling out the Healthy Lifestyle Questionnaire or by calling (800) 532-2227 to request one. Filling out the questionnaire will help NCP determine what services are best for you.   • Depending on your household income, you can afford preventive screenings through one of the three programs. Or if you have health insurance through your employer, you may be able to get free prevention services through that.

  • If you have low to moderate income you can apply for Medicaid Expansion/Heritage Health Adult. If you don’t meet the income requirement, you can apply for the Every Woman Matters Program and The Nebraska Colon Cancer Screening Program to get access to free services.

  • Heritage Health is the managed care program that manages Medicaid services in Nebraska. Heritage Health Adult (HHA) is part of the overall Heritage Health program. HHA covers those eligible for Nebraska’s Medicaid expansion. Those eligible for Medicaid in the Heritage Health Adult category will be enrolled into a Heritage Health plan to manage their benefits.

  • Medicaid Expansion is also referred to as Heritage Health Adult (HHA). It means that Nebraskans 19-64 years of age who earn up to 138% of the federal poverty level are now eligible for Medicaid Expansion benefits.

  • Heritage Health Adult covers Nebraska residents, ages 19 through 64, whose income is at or below 138% of the federal poverty level. For 2022, this is an annual income of $17,774 for a single person or $36,570 for a household of four. Apply online here.

  • All individuals eligible under Heritage Health Adult receive full state plan services, which includes medical, behavioral health, dental, drug, and vision.

  • Apply for Medicaid Expansion/Heritage Health Adult Online or visit a local DHHS office to do it in person. If eligible for Heritage Health Adult, transportation to appointments is part of covered services. Every Woman Matters and the Nebraska Colon Cancer Program do not offer this type of assistance.

  • Fill out this questionnaire and answer every question to the best of your ability! Reminder: you must not have health insurance that would pay for preventive services. If you are able to get services through one or both of the programs, you’ll receive information in the mail. Don’t make an appointment with your healthcare provider until you get this information.

  • You may already have preventive screenings covered through your employer, free of charge. Review your insurance first. If not, you may be eligible. Review eligibility requirements above and apply today.
